- Athletic Pre-Participation Physical Examination Form
- Required for participation in all sports, including tryouts
- Must be signed by the child’s personal physician at the time of the child’s physical examination
- This is the state-approved form; no other physical examination form will be accepted
- Valid for one year from the date of the physical
- Must be certified by our school physician before the child can participate in a sport; certification is done three times per year, prior to each sports season (fall, winter, spring)
- Deadlines for submission are hard deadlines and cannot be missed
- Athletic Participation Health History Update Form
- Required for participation in a Woodglen sport when the child has a valid Athletic Pre-Participation Physical Examination Form on file with the district which is more than 60 days old and less than 365 days old
- Must be signed by the child’s parent or guardian; does not need to be signed by the child’s physician
- Before completing this form, contact Diane Schneck, Woodglen School Nurse, to see if your child has a valid Athletic Pre-Participation Physical Examination Form on file with the district
- Using this form without a valid and certified Athletic Pre-Participation Physical Examination Form on file with the district could result in missing a deadline and therefore having your child ineligible for athletic participation
- Interscholastic Athletics Agreement - Required for participation in interscholastic athletics. Must be signed by student and parents/guardians.
